Occupational Objective
Robert Johnson's current occupational objective is Vocational Evaluation.
He asks that this pass be approved with a starting date of 04/01/94 and
continue for 6 months with an end date of 10/01/94.
Narrative
Robert will begin an evaluation program through Southwest Mental
Health Center. He will go through a series of transitional employment
opportunities designed to increase his awareness of different jobs and
assist in the determination of a feasible occupational objective. He
will attend a support group designed for persons with psychiatric
disabilities interested in returning to work. His PASS will pay for his
program fees, transportation, health insurance, support group and basic
phone services.
His therapist at the Mental Health Center (232-1389) is Kitty Makee
and his vocational coordinator is Peter Hochman.

The Social Services cost is the fee for assisting Robert to find a
feasible occupational objective, developing necessary documentation,
working with other disability systems (e.g. medicaid), establishing a
PASS audit procedure, and preparation of needed amendments within the
first 60 days following PASS approval.
